We can collect information in the following ways:

• Information provided by patients/ parents/ guardians, during consultations/ appointments or via phone/ email.
• Information provided by other healthcare professionals, orthotic and shoe laboratories, and footwear companies.
What type of information we collect
We collect and store any information you provide to us in the form of emails, letters, questionnaires, verbal conversations, forms and assessment notes. This information can include your “personal information”, including your first and last name, address, phone number, e-mail address, medical history, details of your treatment and any other personally identifiable information you provide.
Katherine Thompson does not collect and store any “billing information” except insurance details. Any information given to us for billing purposes ie; card details for over the phone payments, is directly inputted into squareup.com (an online card payment service) which does not store card information after use.
Why we collect your information
This information enables Katherine Thompson to provide professional service to patients and to work in partnership with other professionals. Non-identifying information may also be used to review the services provided and to assist Katherine Thompson to continue to provide a high level of support and care to her patients. The information that you are asked to provide, and the reasons why you are asked to provide it, will be made clear to you when we ask you to provide it.
Information we collect from third parties
We may also receive personal and non-personal information about you from our colleagues in the NHS and the private sector ie: consultants, physiotherapists, GPs and insurance companies, which will be added to your patient file.
Storage of information
Katherine Thompson will keep in contact with you via phone, email or letter. This is an essential part of assessment and treatment and you will only be contacted for the benefit of your health and wellbeing in relation to Podiatry. Following your initial assessment/s and treatment/s we will contact you periodically (usually annually for Adults over 18), usually by email (by post for patients who prefer or have no email access) as part of the ongoing review process of your health and wellbeing. If we have not received any contact or response from you in three years, your records will then be archived. Hard copy archived records will be shredded and destroyed after ten years and all electronic information deleted after ten years of no contact.
If you are under 18, an at least annual review appointment is usually required. Katherine Thompson will continue to keep in contact with you to arrange these reviews. If Katherine has not had communication from you within three years, your records will then be archived but under 18 records are not deleted until the patient reaches the age of 28 (ten years after reaching age 18/ adulthood) as well as ten years of no contact.
Why we collect personal information
We collect personal information to help us deliver services to you, including:
the provision of any product or service you have purchased from us
formulating diagnoses and treatment plans
answering any queries or requests you have regarding treatment, products or services
making information available to appropriate third parties where necessary, and in the event of an emergency

Katherine Thompson will not contact you in a marketing capacity, however may notify you of changes to her products or services applicable to your health and well being. Your information will not be shared with anyone else for the purposes of marketing.
When we may disclose information to Third Parties
Katherine Thompson may disclose personal information to third parties including NHS and private health colleagues, orthotic and shoe laboratories, and footwear companies as well as to medical secretaries. We may also disclose personal information as part of due legal process and as otherwise permitted or required by law. This is explained in the consent form to be completed prior to assessment and treatment.
Katherine Thompson protects the information provided by you from access by unauthorized persons and against unlawful processing, accidental loss, destruction or damage. Such measures include password-protection, and firewall and intrusion detection. Contact with you or other professionals via email will be encrypted and a password to access the email is required. Katherine Thompson has taken appropriate safeguards to require that your personal information will remain protected in accordance with this Privacy Notice. We have implemented appropriate safeguards with our third-party service providers and partners and further details can be provided upon request. If at any time there has been a breach regarding the security of your information Katherine Thompson or her secretaries will report this to the ICO and yourself within 48 hours.
